Problem accounts are a huge and growing dilemma for Health Professionals in private practice in South Africa. There are unfortunately some people who make use of medical or other services and they make a habit of not paying for these services.
Problem accounts can make up to 15% of a medical specialist’s practice. This can be substantially higher in some practices. Over a 25 or 30 year professional career this would have contributed significantly to retirement benefits.
An example would be the "affluent" business executive who visits a general practitioner for an inguinal hernia. The general practitioner refers the patient to a general surgeon who books the patient on his operating list after a consultation with a cardiologist for ischaemic heart disease. A radiologist does a chest x-ray as part of the pre-operative workup. The operation is performed and the patient is discharged without any complications. The patient’s medical insurance settles the hospital bill and pays the patient directly for the accounts of the referring and/or assisting general practitioner, surgeon, anaesthetist, cardiologist, radiologist and physiotherapist. The patient does not settle any of these accounts. This senario repeats itself two years later when the wife undergoes a hysterectomy or the son undergoes an emergency appendisectomy on a Saturday evening with medical services provided by different medical practitioners.
